Order Form and Mailing Instructions

· Please fill out our “printer friendly” order form.  Forms can be faxed or sent with your quilts.

· Please include a copy of your order form with your quilt.

· Remember to insure your quilt – We will insure your quilt for at least the same amount when we return your quilt to you.

· When we receive your quilt, we will contact you and make arrangements on how best to send you your invoice. 

· When the work is completed, and we have received your payment, we will ship your quilt back to you!

· There will be a $25.00 charge for returned checks
